Therapists - Copy this React, Tailwind Component to your project

I have to create appointments form where therapist will be select from dropdown and date and time will be select from datetime picker and medical branch will be select from drop down these fields are for one appointment if patients needs multiple appointment there should be a button for add appointment if user press it above filed will show again in bottom with line divider and user can fill next appointment and so on. Here i have to clarify you that therapist and branch is required for first appointment and for other appointments it's optional date and time is required for all, and use Formik to validate form Use MUI Icons, date and time picker and tailwind css only
