Service Provider Dashboard - Copy this Html, Bootstrap Component to your project

Create a page that will have input field "from date" , "to date", type select "service provider" and submit button. this will fetch the data from backend server and show the result as table. at the end of each row, there will be option to "view" as eye icon and approve button.
