Create a checkout Page, similar to woocommece plugin checkout page where should be a customer billing details form, Shipping Details Form, and then checkout payment form. But All the three form should be in a step by step process. I mean Initially the customer billing details form should appear, and then continue button to go to the shipping form, and then the checkout payment form.
