Confirmation Modal - Copy this React, Tailwind Component to your project

I want to make a confirmation modal for the supervisor before he unsubmits a particular form of an employee. The confirmation card should pop up with the title "Confirm Unsubmission" the description must be "Ram Umarkar's form for 14/11/2024 will be unsubmitted". Also there must be a toggle switch to decide whether to send email to the employee or not about his form unsubmission. And there must be a cancel and a confirm button at last. Please do any further improvements in the text if required. And make a very nice dark themed UI for it.
