Dynamic Form Card - Copy this Html, Bootstrap Component to your project

Create a Form which is dynamic change form card on basis of screen size. Form input fields are First Name , Last Name , username , country , state , city. Placeholder of input should present in input field when user click on input placeholder value move on upper side of input box. this ford contain in card and on top of card have option of dark mode / light mode. background of color should change color after every 3 second (should be smooth change) and if user choose dark mode and colors selection for background of card should be dark and if light mode then light colors
