Bank Management Table with Pop-up Form

Build jsQuary table with button named "New Bank" so if user click on that button it should open pop up form. in that form there should be Type withdropdown Current, Cash Credit, Loan Account, Over Draft, Credit Card, Saving and another input field names should be like this : Ledger Display Name, Bank Name, Bank Branch, Account Number, Account Name, Bank SWIFT and there should a checkbox named "Active" which is auto marked also provide me code focusing on ux design.
