Login Page Component - Copy this Html, Bootstrap Component to your project

I need a code for a simple login page, when i type this url: http://clientw10/rubrica/admin i wanna see a form with a requesto of just user and password, after the credential verification i have to see the admin page, my project use CodeIgniter and Boostrap
