Feature- Rich Modal - Copy this Html, Bootstrap Component to your project

In a modal add a button named Add. when it is clcked it renders or show a input text named Observation and button named Cancel and a button named Ok. When either the Ok or Cancel button are clicked the rendered input text and buttons must hide.
