The modal should contain the following: On the left side of the modal, there will be a subtitle called "Provider" and below this subtitle, there will be a Dropdown Select bar with a placeholder that contains the following text: "Select a provider." When clicking on this bar, it should allow interaction and selecting one of the registered options (even if no options are registered yet).
