Subscription Form - Copy this React, Tailwind Component to your project

I want ot make subscription form there are different different plan 49 and 149 plan is one and second is 549 so i want that if user so i want if user click on 49 subsciption the show the options in form 49 and 149 and if user click on 549 one off plan then amount should fill automatically.
