Two- Column Form - Copy this Html, Bootstrap Component to your project

I want to create a form that has two columns. The left column will be 1/4 of the right column. The left column will have 8 inputs and right column will have a table that will display rows of data. The row data has 5 fieldsthat can be entered by pressing a add button.
